Reports

This reference page provides information about the template reports available in Komodo Care Connect. You can either use the templates straight out of the box or clone them to create custom reports.

This folder contains reports of the Case records owned by the current user, including care plans, activities, missing information, and requests.

A report of all of the user's CarePlan Case records. The care plans are grouped by the Therapy Access (mvn__PJN_Therapy_Access__c) field of each Case record.

A report of all of the user's CarePlan Case records. The care plans are grouped by the age (in months) of when each Case record was opened and then closed.

My Care Plans by Physician

A report of all of the user's CarePlan Case records.. The care plans are grouped by the Physician (mvn__PJN_Physician__c) field of each Case record.

My Care Plans by Status

A report of all of the user's CarePlan Case records. The care plans are grouped by the Therapy Status (mvn__PJN_Therapy_Status__c) and Therapy Sub-Status (mvn__PJN_Therapy_Sub_Status__c) fields of each Case record.

My Closed Activities

A report of all of the user's closed PJN_Activity and PJN_Missing_Information Case records.

My Closed Activities by Age

A report of all of the user's closed activities PJN_Activity and PJN_Missing_Information Case records. The activities and missing information cases are grouped by the age (in days) of when each Case record was opened and then closed. Out of the box, the Age Category buckets and ranges are:

  • Less Than 1 Day

  • 1 to 3 Days

  • 3 to 7 Days

  • 7 to 14 Days

  • Over 14 Days

A report of all of the user's closed PJN_Activity and PJN_Missing_Information Case records. The activities and missing information cases are grouped by their programs and parent care plans.

A report of all of the user's closed PJN_Activity and PJN_Missing_Information Case records. The activities and missing information cases are grouped by the Subject field of each Case record.

A report of all of the user's closed PJN_Request Case records.

This folder contains reports of the different information that any care plan may capture, including, but not limited to, expiring consent, member plans by payer, and patients by age.

A report of all Assistance Program (mvn__PJN_Assistance_Program__c) records. The assistance programs are grouped by their record types and the Outcome (mvn__PJN_Outcome__c) field on each record.

Care Plans without Consent

A report of the care plans that do not have related Consent (mvn__PJN_Contact_Point_Type_Consent__c) records. The care plans are grouped by the owner of each Case record.

A report of all of the closed PJN_Activity and PJN_Missing_Information Case records. The activities and missing information cases are grouped by the owner of each Case record.

A report of all Consent (mvn__PJN_Contact_Point_Type_Consent__c) records. The Consent records are grouped by their parent care plans.

A report of all Consent (mvn__PJN_Contact_Point_Type_Consent__c) records. The records are grouped by the Capture Source (mvn__PJN_Capture_Source__c) field on each Consent record.

Consent by Type and Status

A report of all Consent (mvn__PJN_Contact_Point_Type_Consent__c) records. The records are grouped by the Data Use Purpose (mvn__PJN_Data_Use_Purpose__c) and Privacy Consent Status (mvn__PJN_Privacy_Consent_Status__c) fields on each Consent record.

Coverage Investigations by Care Plan

A report of all Coverage Investigation (mvn__PJN_Coverage_Investigation__c) records. The coverage investigations are grouped by their parent care plans.

Coverage Investigations by Outcome

A report of all Coverage Investigation (mvn__PJN_Coverage_Investigation__c) records. The coverage investigations are grouped by the Access Type (mvn__PJN_Access_Type__c) and Outcome (mvn__PJN_Outcome__c) fields on each record.

A report of all Coverage Investigation (mvn__PJN_Coverage_Investigation__c) records. The coverage investigations are grouped by the Status (mvn__PJN_Status__c) field on each record.

Expiring Assistance Programs

A report of all of the expiring Assistance Program (mvn__PJN_Assistance_Program__c) records that are expiring. "Expiring" means that the Expiration Date (mvn__PJN_Expiration_Date__c) value on the Assistance Program record is 90 days or fewer from the current date.

Expiring Consent by Case Owner

A report of all of the expiring Consent (mvn__PJN_Contact_Point_Type_Consent__c) records whose Privacy Consent Status (mvn__PJN_Privacy_Consent_Status__c) field equals Opt In. "Expiring" means that the Effective To (mvn__PJN_Effective_To__c) value on the Consent record is 90 days or fewer from the current date. The records are grouped by the owner of each Consent record.

Expiring Prior Authorizations

A report of all of the expiring approved Prior Authorization (mvn__PJN_Prior_Authorization__c) records. "Expiring" means that the Expiration Date (mvn__PJN_Expiration_Date__c) value on the Prior Authorization record is 90 days or fewer from the current date.

Fulfillment Requests by Care Plan

A report of all Fulfillment Request (mvn__PJN_Fulfillment_Request__c) records. The fulfillment requests are grouped by their parent care plans.

Fulfillment Requests by Type

A report of all Fulfillment Request (mvn__PJN_Fulfillment_Request__c) records. The fulfillment requests are grouped by their record types.

A report of all Fulfillment Request (mvn__PJN_Fulfillment_Request__c) records. The fulfillment requests are grouped by their record types and the Status (mvn__PJN_Status__c) field on each record.

A report of all Member Plan (mvn__PJN_Member_Plan__c) records. The member plans are grouped by the Payer (mvn__PJN_Payer__c) field on each record.

A report of the Member Plan (mvn__PJN_Member_Plan__c) records that have related Prior Authorization (mvn__PJN_Prior_Authorization__c) records. The member plans are grouped by their parent care plans and the Outcome (mvn__PJN_Outcome__c) field on the Prior Authorization record.

Missing Information Activities

A report of all PJN_Missing_Information Case records. The missing information cases are grouped by the owner and Status field of each Case record.

A report of all of the open PJN_Activity and PJN_Missing_Information Case records. The activities and missing information cases are grouped by the owner of each Case record.

A report of all of the open PJN_Request Case records. The requests are grouped by the owner of each Case record.

Overdue Activities by Owner

A report of all of the open overdue PJN_Activity and PJN_Missing_Information Case records. "Overdue" means that the Due Date (HealthCloudGA__DueDate__c) value on the Case record has passed by one or more days. The activities and missing information cases are grouped by the owner of each Case record.

Overdue Requests by Owner

A report of all of the open overdue PJN_Request Case records. "Overdue" means that the Due Date (HealthCloudGA__DueDate__c) value on the Case record has passed by 1 or more days. The requests are grouped by the owner of each Case record.

A report of all patients. The patients are grouped by their age.

A report of all Prior Authorization (mvn__PJN_Prior_Authorization__c) records. The prior authorizations are grouped by the Type (mvn__PJN_Type__c) and Outcome (mvn__PJN_Outcome__c) fields on each record.

Program Stages by Duration

A report of the Problem (HealthCloudGA__CarePlanProblem__c) records in a care plan. The problems, or stages, are grouped by their names and are measured by the number of days they have been or were open.

Program Stages by Status

A report of the Problem (HealthCloudGA__CarePlanProblem__c) records in a care plan. The problems, or stages, are grouped by both the name of the related Care Program (CareProgram) record and the Status (mvn__PJN_Status__c) field of each Problem record.

A report of all PJN_Request Case records. The requests are grouped by the age (in days) of when each Case record was opened and then closed. Out of the box, the Age Categories buckets and ranges are:

A report of the PJN_Therapy_Status_Update Event (mvn__PJN_Event__c) records whose New Therapy Status (mvn__PJN_New_Therapy_Status__c) field equals On Therapy and whose New Therapy Sub-Status (mvn__PJN_New_Therapy_Sub_Status__c) field equals Triaged to SP. The therapy status update events are grouped by their parent care plans.

A report of all PJN_Therapy_Status_Update Event (mvn__PJN_Event__c) records. The therapy status update events are grouped by both their parent care plans and the date that each event was created.
