Glossary
- Activity
An activity to be completed to progress a patient through their care plan.
- Care manager
The team member who supports the patient through their program journey.
- Care plan
A record representing a patient’s journey through a program.
- Care team
The group of people playing a role in a care plan, such as the patient, caregiver, physician, and staff member. This also includes any other relationships that a patient has with individuals and organizations in Komodo Care Connect.
- Case
The name of the Salesforce object that serves as the baseline for care plans, requests, and activities.
- Enrollment
A patient’s enrollment into a program, including the processing of a patient’s enrollment form and the creation of a patient’s care plan for a program.
- Entity Relationship Diagram (ERD)
A visual representation of how entities in Komodo Care Connect, such as objects and metadata types, relate to one another.
- Event
A record representing either an inbound response or update from a third-party provider (e.g., a specialty pharmacy) or a key change in a patient’s care plan.
- Fulfillment request
A record representing an outbound request to a third-party provider (e.g., a specialty pharmacy).
- Note
A note related to a specific activity or the patient’s care plan in general.
- Patient Journey Navigator (PJN)
The former name of Komodo Care Connect.
- Program
A pre-defined patient journey through a specific treatment.
- Request
A record representing an inbound request for some type of information or for a certain process to be completed (e.g., an enrollment form). This is typically submitted via email, fax, or phone.
- Stage
A collection of activities for a particular phase in a care plan.
- Timeline
A visual representation of key records relating to a patient’s care plan(s), plotted on a timeline.
